Refugee, Great article, I couldn't agree more. Access is a huge issue. When I left primary care I'm a little over a year ago, I could not direct many of my 1000 or so patients to a new PCP as there simply weren't any I knew of taking new folks. At the time I was the only new, young, primary internist in the entire city that I knew of... and we had two IM residencies that graduated almost 20 new docs a year! But, alas, all went to hospitalist jobs or fellowships. I soon found out why and followed suit.
